HOW TO GET NEW PAN WHEN IT IS LOST

Instructions For Filling Form
For Lost Pan card fill up Application Form & submit in authorised agency NSDL as well as 96.00 rs.
(a) This Form is to be filled readably only in BLOCK LETTERS using BLACK INK.
(b) Ensure to fill the correct 10-digit PAN.
(c) 'Individual' applicant must attach his/her color photo (size 3.5 cm x 2.5 cm) with the form. Should there be any amendment/change to be made in the info printed on the PAN card (provide as item no.s.1, 2, 3, 5 and 6 in the change request form). You cannot staple or clip the photo in the form. (The transparency of image on PAN card will be contingent upon the quality and transparency of photograph attached to the form.)
(d) Initials / Left thumb impression is to be made within the specific box in the form ensuring it does not cover the photograph. In case a mark is found on the photograph in a way it challenges the clarity of the applicant’s face, the application will not be entertained.
(e) If the Thumb impression is used, ensure it is solely attested by a Magistrate or a Notary Public or a Gazette Office with official seal and stamp.
(f) To make any amendments/changes in PAN data, fill every panel of the form and tick box on left side of the matching line. Enfold previous PAN card (in original form), if issued, or facsimile of any document/letter granted by the Income Tax Department indicating PAN should there be any change in the particulars inscribed on PAN card (i.e. item nos.1,2,3, 5 & 6).
(g) To get a new PAN card issued without new details - In the instance you get hold of PAN but without a PAN card and looking to obtain a PAN card or reinstate a destroyed or lost PAN card, fill every column of the form ensuring you do not tick any of the boxes on the left side.
In case your PAN card is lost, attach a FIR copy with this lost PAN card form. If they are individuals, do attach a photograph with the form.
(h) Possessing more than one PAN card is an offence. In case you own over one PAN card, please fill the particulars in Item No.10 of this form and submit it.
