14 December 2010 refer the format as per annaul return to be enclosed as a attachment. prepare in that format.

15 December 2010 If the shareholders are few you can show the details in 20B itself and no need to attach a separate list even though you will have to say "yes" to the column "Whether Sharesholders list is attached?" appearing in Form 20B.

Only if the number of shareholders are huge you will have to attach a separate sheet incorporating the following details:

Name of the Shareholder:
Father's Name:
Type of share : Equity/Preference/Debentures/Stock
No. of Shares held:
Amount per share:
