Late submission of form 109 (please help)

Dear CAClub Members, I have taken transfer from articleship on 16th july 2016,But due some reasons i was not able to submit form 109 within 30 days.Now i am very much worried that exactly what should i do to get it accepted,so please help me in this by anserwing the following questions:
1.Do I have to pay certain condonation fee and write a condonation letter in this respect ?
2.If I have to write a condonation letter what resons should I state in my letter so that it will be accepted ?
And,
3.If any other action is required to be taken please let me know ?
plz plz help me, plz reply its urgent .Thnks in advance

you can write the REAL reasons for the delay and request for condonation

Why not, if it is the real reason....write it....Also write that henceforth you will take utmost care in following the rules....Write that ignorance of law is not an excuse, but still you were ignorant of the same.....please excuse....
