Dear Sir,
i want submit income tax return hard copy ( other than online) so kindly tell me the appropriate form of Return for income from LIC commission.& from where shall i collect it.

You have to file ITR4 and the same can be downloaded from http://www.incometaxindia.gov.in/download_all.asp. And also download the Acknowledgement Form ITR V from the site.
