what is the procedure to apply for industrial training ?

u need to file form 104 and u have to registered with those co which is registered with icai for imparting industrial training .

you need to register with a company in india which has been affiliated by ICAI for providing industrial training options
the process is that first you will give an intimation to your current principal for your intention to go for industrial training atleast 3 months in advance.
when you have to start industrial training, you would execute form 109 with current principal and Form 104 with the company and submit both to ICAI office.

further u can do it only when ur in ur last year of articleship and the duration shall be between 9-12 months only.
