If case of demise of an assessee, what is the procedure to claim income tax refund by the widow of assessee ?

Return of Deceased will be filed by the legal Heir of assessee and thereafter, the income will be taxed in the hands of respective legal heir and TDS will also be claimed by that legal heir.
Also, the refund may be issued by the department in the name of legal representative or the legal heir whoever is filing the return.
