Freight certificate

Hello All,
I am working as accounts executive with manufacturing company in Pune. We received one consignment from Germany and which is on sample basis. Our supplier is going to pay the freight (custom) charges for the same. But custom is demanding freight certificate from our supplier. So I want to know whether there is any standard format for the same and if yes can anybody send the format to me ?
