Format of statement of affairs

Hallo Sir, I am a salaried personal, I have received a Notice U/s 142(1) & 143(2) from Asst. C.I.T for the A.Y.2008-09 where they required some documents, along Statement of Affairs,

I have collected all those documents except Statement of Affairs, I don’t know what does it mean and how to prepare it.

I would be happy if any of our experts can provide me the format of Statement of Affairs.

14 September 2011 A balance sheet format would do.

Also one thing the time limit for completion of assessment u/s 143(2) was 31st December, 2010 and so now the case is time barred and order should have been passed till now. Otherwise it would be an illegal order.
