Format of Rectification Application u/s 154 of I.T.Act

Can any one provide me the format of Rectification Application being filed in response to an intimation u/s 143(1) of I.T. act to conerned I.T.O.
The asseese has filed regular I.T. return relevent to Asst.Year 2009-10 showing income below taxable limit & claiming TDS refund vide duly Acknowledged by Dept. on 30/06/2009.
However he has not given his Bank Account details for refund purposes.The Return was filed manually to concerned Dept.
