format of medical certificate

is there any fixed format of ICAI of medical certificate in case of transfer in 2nd year of articleship?
i visited the govt hospital for the same and showed them my medical reports, they write in casual manner that i should avoid walking and travelling and standing for long.
they didnt mention the time limit.
my ques is will it be approved by ICAI?
as they wrote all these lines in a plain and simple format on the hospital letter head with stamp...
