Formalities to change the name of a Pvt company

Can anybody let me know the formalities (forms, fees, resolutions,etc.) to change the name of a Pvt Ltd Company.

09 April 2009 First apply for the new name which you want.

Once you get the confirmation that the name is avaialble then

Conduct a Baord meeting bring in the subject name change and reasons for the same.

Conduct EGM and get the approval of the shareholders for the change of name.

File Form 23 for the resolution and also corrected memorandum and articles of association incoporating the new name.

Surrender the old certificate of incorporation and get the ammended certificate of incorporation.

Then inform your bankers and publish the same if your management required.
