In above form a attachment required for specimen signature that is Form INC 10
I searched it on MCA site but I could not fined the same.

Format of INC-10 available in Companies (Incorporation) Rules, 2014. Take the same in stamp paper.

INC-10 is for an attestation of photo and signature. The attestation has to be done by a Notary. No need of stamp paper.
