Form 27A for corrected Quarterly Returns - 26Q

What is the format of Form 27A that has to be filed with the corrected Quarterly returns 26Q (C5 type of correction)? The original Form 27A shows control totals which will not be reflected in the statistics report. Please Advise.

there is no other proforma for form 27 A. it will be automatically generated from the tds software which u are using and take statistics report print out both are sing and submitted to nsdl
