For transfer of articleship after 1 year

22 July 2014 i living in chandigarh which is 250 KM from my home town from last 6 months and i have completed my articleship 1.5 years
due to bad health i want transfer and want to shift to my home town i have medical certificate of rest period of 1 month from govt hospital
what should i do to take transfer

22 July 2014 then first on plain paper with all evidence of new address u need to take permission from icai and only when they permit u u need to file form 109 for transfer else it will be rejected.
