22 November 2013 Dear Sir/Maa'm

I have done my articleship for 10 months now and I wish to terminate the same due to some personal resons.

Please guide what all form/applications I need to get signed from my principal.

I am confused between Form No. 108, 109, 119.
If any other form/application is also required then please guide.

Please guide regarding the time limit to send the same and is there requirement of any affidavit?

22 November 2013 you need to take form 109 and duly fill it prpoperly in all coloumns
