What is the procedure to open Letter of Credit (L/c)? Is there an prescribe form of bank?

IN FIRST, EXPORT SALES CONTRACT SIGNED BY BUYER AND SELLER.
IN SECOND, THE LETTER OF CREDIT OPENED BY THE BUYER .
PROCEDURE:-
I REQUEST LETTER GIVEN IN BANK IN BEHALF OF EXPORTER .
