Is it compulsory to get signature from a chartered accountant in the cpt exam form. i want address where that form should be dispatched.

Yes, Signature of the Member of ICAI or Gazzetted officer's signature is compulsory on CPT exam form and the form should be dispatch to the New Delhi following address
