Dear Sir how to calculate the esi deducation and what form will be maintained

ESI up to 10000 salary @1.75% on employee
and on employer 4.75% to be deduct.
deposit by esi challn in account no.1.
and on employer 4.75% to be deduct.

Ram Avtarji is correct. However 1.75% from employee to be deducted and employer to contribute 4.75% from his pocket.
