ESI-due dates and non-cmpliance

Dear forum, I want to know the due date of payment and returns of ESI in Andhra Pradesh and Interst/Penalties for non-compliance with the due dates.

21st of the following month.
31 October 2008
It will invite penalties and interest.
31 October 2008
21st of next month for employeecontribution in order to avoid deemed income as per sec.36 of the IT act.
Even employer has to remit within 21st of the following month but he can pay within due date of filing return.

There is no panality for late submission to form No- 1, but it is need to write a letter that we could not submit the ESI Declaration forms on time because __________________.
