Documents Submission While Scrutiny

We have submitted Bunch of Documents while Our Assessment under scrutiny assessment. Now commisioner has opened case U/s 263 and during proceedings, he has found that some of the annexure submitted while scrutiny is not there in his record, so it is deemed that we have not submitted.
As you are aware that during Scrutiny assessment, No office gives the acknowledgement for the documents submitted, They only make note of some of the points in their proceeding sheet.
What Stand We should Take for that.

It is always safe to give one set of documents through the counter and get acknowledgement. If any document is given without acknowledgement, the giver is at risk.

RIght Sir, I do agree. But Now what should we stand for . It is past mistake, And Now onwards we will take care. A.O., while passing Order could have asked the details again, He has passed the Order and Now again U/s 263.
