Disclosure of Form 15G in TDS Return

When i am filig my TDS Return there are so many cases where form 15G received.
Can anybody help me to come out of the confusion that whether Form 15G is required to be disclosed in TDS Return or not.
Please reply.

Yes, it has been mentioned at reason for lower deductin of tds.
