Compounding of offence

Friends i have received notices from MCA against various sections of companies act. I have replied to this notices to the department, now i have been told to file petition for compounding of offence. Plz help me out as wat is the procedure, whr to file, format all detailed information.

For compounding of offence you are required to file E-Form 61
