we have a/c in co-op bank closed the a/c we need certificate from bank. pls provide any perticuler format for this.

There is no such format for it. Bank gives you the same on there latter head

No such format is required.

Simply bank will write a letter on its letter head that the account number such and such maintained by such and such person having following address has been closed on and from this date.
