Clarification on giving single proof for three verification

15 September 2011 is single copy of pan card self attested is sufficient as attachment for proof the name change, date of birth change & intimating pan details in annexure-2 of DIN-4 or we have to give seperate proof for each change

what is notification number of DIN-4?
