08 September 2011 Can anyone give me a Format of application form for registration of a CHARITABLE TRUST in Andhra Pradesh or a general application form for registration of Charitable trust.

Please help me out its urgent!!!!!

I have gone through the link u mentioned above, but i have not the exact info that i need, because i want the application format in general, the one which is present in the above link is under Bombay Public Trust Act,
so please provide me the format which would be common for all states,
