Can you please suggest the documents to be attached alongith 'Changes in PAN data ' form if anyone , after marriage , wants to change her name (surname) and address.

husband's ID for change of sir name, if not mentioned in the marriage certificate. if full name is mentioned in marriage certificate no other document is required. if address is also to be changed address proof for new name is also required. send pan correction form alongwith photograph and old pan card copy
