Yes its ok . but you need to submitt Gejetted Officer certificate with that

There is no requirement for date of birth proof. And the format of ration card that you specified is common in most of the states. Don't worry, your present ration card is sufficient for this purpose. Their requirement is of proof for- name, father's name, and address only. No other requirements. And in case of married woman, even father's name proof is not required.
