What do you mean by cost plus billing

Cost plus billing means Billing is based on actual cost of the goods/services plus a profit margin mutually negotiated and settled betwen the parties concerned.

Milestone billing is nothing but stage wise billings. When the work is prolonged in nature such a billing is resorted. Events that should trigger billing are agreed at the tie of agreement and implemented.
