I'm eligible for may 2017 IPCC exam but because of some reason I'm thinking to give exam in Nov 2017
Is may 2017 attempt is counted???

Yes..It would be counted as one attempt

If you do not fill the exam application form, then it will not be counted as an attempt. However, the count is considered mainly during campus interview time.
