Articleship completion certificate

hello
I completed my articleship on 2 sep 2014 and I haven't gotten articleship completion certificate from institute which I have to attest in the exam form for appearing in may 2015 final exam.when I enquired in my city's institute office the official person said I have to send a request in written to Kanpur,but he didn't tell me which department and what to write?please help me in this regard.

there is no need to write such letter only u have to fill up form 108 duly signed by ur ca and completely filled up.
