Dear sir /Madam
I finished my 2nd year of articleship and due to medical issues I want to take termination.
So pls suggest whether I can send my request letter along with form 109.
Because prior approval needs some time to get back.

no,,,,first u need to send an application on plain paper with all the proof of medical grounds and then if icai approves then file form 109.
