I have taken membership of ICAI. Now I want to take certificate of practise. What is the procedure for the same. What are the forms to be submitted?
Can I form proprietorship is some other name, instead of my own name?

You have to apply in Form 6 to take COP. If you want to start Proprietorship Firm than you have to apply for Firm Name in Form 117. After you get your Firm Name approved from ICAI, you will get e-mail for the same. Apply in Form 18 for registration of your Firm within 30 days of approval of your firm name. You are correct Form 6(along with fees) is to be submitted at Regional Office and Form 117 at Delhi.
Once you get the mail from ICAI regarding approval your firm name, send Form 18(attach print of mail approving firm name) at your regional office. Otherwise Condonation fees would be applied.
