I have to send my form no.109 but i dont know the address. So, let me know the address.

You have to send it to your regional center.
Ie, if your Student Reg no starts with SRO- Southern Region
WRO- Western Region
NRO- Northern Rgion
CRO- Central
ERO- Eastern Region
