Address for posting articleship termination form

Hello All
I am from Amritsar , Punjab (Northern Region of ICAI) . I have completed my articleship, and now i have to submit the termination form within 30 days which is 30th March. Kindly provide me the address where to post the form (For Northern Region) and is there any other document , which has to be submitted along with the form 108 or only the Form we have to submit. Kindly provide me the info As soon as possible. Thanku :)
