Can you please give me a draft bank letter for AD code?

Dear Sir,
after getting the IEC CODE, exporter must submit request letter / email to their banker for issuance AD CODE (addressed to custom port of export where wish to export) :- banker will type the format :-
Format on Bank Letter Head
TO WHOMSO EVER IT MAY CONCERN
This is certified that M/S _____________________________heaving regd. Office at C______________________________________are maintaining their Current Account No. _________________, satisfactory with our bank.
