26as tds not reflecting in demand

what ever we pay the tds amount reflect in 26as based on that we prepare income tax filing but demand is come for the purpose of out standing amount is pending.some of the tds amount reflected in demand but no full why?

MAY POSsible you have not claim the TDS amount in ITR, or after filing of return some tds entery reverse by the deductor.
