Usl cpe form submission

Can anyone guide me in submitting unstructured cpe credit self declaration form.. i.e whether the said form needs to be submitted via post or by e-mail? if by e-mail can u gimme the mail id pls?
This form can be submitted via post to one of the four decentralised offices or can be submitted to the local ICAI branch for forwarding it to the respective decentralised office. It would be advisable to send CPE information by post or by hand than email.
