Urgent help needed regarding exam form

Hi
I wanted to know whether stamp of CA is required or is it sufficient if the CA signs and puts his membership number in exam form and Articleship completion certificate.
kindly reply ASAP.

Practising CA put their seal and sign the examination application form. Non practising members indicate their Membership number and sign the form, that holds good as well.
