Lost mark sheet for ca ipcc group-1

I had lost my CA IPCC statement of marks so i wanted to have a duplicate one. When i was trying to make a online request on icaiexam.icai.org i came along a requirement to upload a scanned copy of handwritten application. Now should i upload scanned copy purely written by hand or first download blank format given on the below link and the fill it.

07 February 2017 After downloading the format, modify details as per your requirement, take a print out of the format, fill it by your own hand writing and sign the application and scan it. Later log on to the Later log on to the portal - https://icaiexam.icai.org and also pay the applicable fees on-line through credit cards.

Note: Payment of applicable fee through internet banking or debit cards is NOT permitted.

No need to submit the print out of the online application or any other documents by post.
