Income tax refund form (urgent)

21 April 2013 Dear Sir

already file ITR but in ITR address mention was wrong.so refund cheque was retuned so i file as per 143(1) to CPC. and also request for refund re issue .but no respond.the next step is apply to AO for that i want form for that?

21 April 2013 according to me there's no form for the same. you have to write a letter regarding the same situation to the department signed by you and attach the copy of return and the correct address proof.
though if you had filed the original return within time than you can first revise it then also attach the copy of same other wise go with the option left with you.

is there any option revise it.
intimation u/s 143(1) is 07/05/2011.
refund re issue ACCEPTED ON 17/11/2012

current refund status in e filling site :refund unpaid
reason :No such Address

can i file request u/s 154 or any other way?
