15 September 2011 Hello experts,

I have to file DIN 4 form for declaration of my PAN number and for the change in my email ID. As required , i have to attach a declaration for the change in my emaid id.

Can anyone please help on the format of the declaration for the change in Email address and Mobile No.

just write on the paper in favour of company covering both existing and new cell no and e-mail id. you need to sign the letter. It should bear the complete adrees of the company and your name, your DIN, and it should bear some date

for format for verification, it is given on MCA Portal.you just click on DIN application from where you download DIN 1, 4 and below Annexure -II is given for the change.
