Application for icai membership

For application of membership, in form 2 check list portion towards the end of the form it is mentioned ID card form duly filled up.. My question is should we send the ID card form along with form No 2 for application of membership or is it only after the membership number is recieved.. coz the id form clearly asks for membership No

U need to send ID card form along with Form-2 at the time of application of membership.
