dear sir Ajay Mishra thanks for your reply singh is not my father last name its a middle name for that i have gave affidavit and fillled the din from as father name first name dharambir singh last name randhir singh (grantfather name) now it again rejected due ti miss match of name filled and evident attached.. i have mention my name as first name ---- jyoti last name dharambir singh plz suggest me how i fill form this time thanks
