Incorporation

Dear Professional Friends,

One pvt ltd company under incorporation -  In this company having one shareholder & director is resident and another one is resident of Malaysia. 

Now it's came resubmission with reason of 

"Attach PAN undertaking of Foreign subscriber"

- we need to attach non-pan declaration for foreign shareholder & director. 

My question is :

In this non-pan declaration, self attestation only enough? 

Or 

Notary and consulate also compulsory?

Kindly let me know. It's Urgent.
