Verification query... please help

Hi Friends,

I have applied for verification online, but while applying I have uploaded the scanned image of Certified copies form and not verification application letter.

In this case what do you suggest whether to refill the verification form again, or will ICAI accept such application?

Because there is no form for verification and ICAI says the letter can be written in any format, just giving out the details of information required, which are present in the form that I have uploaded, only discrepancy being that it is certified paper form scanned image.

Please help me on this if you have any info. I have contacted certain people through email and phone but no one seems to have any idea, they are not replying. Also if you can give details of the person to whom i can contact for help, I would be very thankful to you
