Can a Director be appointed and removed at the same time in a Pvt Ltd Co? Can one DIR-12 be filed for both??

My Queries are
(1) When i file my DIR-12, both additional and deletion can be included in the same form?
(2) What other points ive to note?
din can't accept two times in single dir-12, so u need to file separate firms for induction and deletion
