Income tax refund query

Hi My Company has paid my Income Tax for the year 13-14.

I submitted rebate documents i.e LIC Premium, Education Loan Int. certificate & Rent Agreemnt in this January, beacuse of which my tax my deducted till december.

My query is now related to Income Tax Refund:-

1. How to file ? & where is the form available ? 
2. Where to file ?
3. How much time does it take to get the refund ?

It woudl be of great help, if you can advice on the captioned case.
