Form 112 is compulsory if u r doing any other course.
So in my opinion form 112 is compulsory in ur case.
Also sign and stamp of university is needed.
if u are a regular student, only then sign & stamp of college is mandatory, otherwise only sign & stamp of ur employer is necessary
Form 112 is compulsory and sign with stamp of college only in case of regular course otherwise employer sign is sufficient.
