No documents to be attached. Just get form signed from a CA, preferrably from a ca under whom you have taken articleship.
U need to submit the attested copy of articleship completion certificate (If you have completed the same)
After getting exam form in pdf , Go to second page of pdf you can find the list of enclosures below signatute of prinicipal/member.
Attach the docs wihch reflected there only,no other required.
