go to www.icai.org there u will find a link on online registration fot cpt and all your queries will be solved

copy the above link oin the internet explorer ..you will get cpt registration form.
fill that form with the instructions provided in that.
For registering CPT manually, go to ICAI Chapter of your region, buy a prospectus which contain registration form, and you will find all other details in the prospects.
